Minimum: 34.31 per hour Maximum: 45.03 per hour
The Transition Services Coordinator screens, assesses, coordinates and evaluates the needs of patients and families who cannot function independently in the community. The TSC collaborates and assists the units in expediting discharges for individuals who require community services. This may include arranging home care support and linking patients to the appropriate community agencies.
  • Baccalaureate degree in nursing preferred; however, skills and experience relevant to nursing practice in community sector will be considered.
  • Three years of recent experience in a community setting or acute setting.
  • One year recent Transition Services experience required.
  • Community related experience preferred.
  • Current BCLS-HCP Certification.
  • Active registration with the CARNA.
  • Excellent interpersonal, communication and organizational skills.
  • Ability to monitor financial accountabilities.
  • Proven critical thinking, decision-making skills and assessment skills.
  • Physically able to perform all assigned tasks.
  • Demonstrated ability to perform all duties in accordance with related legislation.
  • Demonstrated familiarity with computer programs required.
