Minimum: 34.31 per hour Maximum: 45.03 per hour
The Transition Services Coordinator in the Emergency Department screens, assesses, coordinates and evaluates the needs of patients and families who cannot function independently in the community. The Transition Services Coordinator collaborates and assists the emergency department in expediting discharges for individuals who require community services. This may include arranging home care support and linking patients to the appropriate community agencies.
  • Baccalaureate Degree in Nursing Preferred - however, skills and experience relevant to nursing practice in community sector will be considered
  • Community related experience preferred
  • Must have excellent assessment skills, interpersonal, communication and organizational skills in order to work with a multi-disciplinary team which includes allied health, physicians and nursing staff
  • Current BCLS-HCP Certification
  • Must have an active registration with CARNA
  • Excellent interpersonal, communication and organizational skills required
  • Proven critical thinking, decision and assessment skills
  • Physically able to perform all assigned tasks
  • Physically able to work in a high paced department
  • Required to perform up to 4 hours of keyboarding (up to 50 wpm) and written documentation per shift while navigating a variety of software programs
  • Demonstrated ability to perform all duties in accordance with related legislation
  • Demonstrated familiarity with computer programs required, must be able to type in pertinent patient information
  • Criminal background check required
