The SHC will adhere to four key pillars of service: Collaborative Practice, Wellness, Patient and Family Centered Care and Innovation. These pillars serve as the key philosophical principles to be used in the development and service of all care to patients and families at SHC.
The Rapid Access Unit (RAU) is a unit that is specifically staffed and equipped to receive medical patients presenting with acute non-critical medical illness from the Emergency Department or directly from a Community Family Physician for expedited multidisciplinary team and medical assessment, care and treatment for up to a designated period of time (4-48 hours) prior to transition home or transfer to an inpatient unit where appropriate. In this model of care, the patients are identified early and resources are loaded upstream in the patient’s journey with a focus on strong coordination and transition back into the community to support more effective patient health outcomes.
