Under the direction of the Manager, Nutrition Services, the Home Care Dietitian is responsible for assessing, developing, implementing and evaluating nutrition care plans for Home Care clients in Calgary & Area. As an integral member of the inter-professional team, the dietitian interprets client needs, assesses requirements and identifies solutions to improve the clients’ nutritional health and well-being. The dietitian consults with other professionals within AHS, communicates with team members, and documents appropriately. The dietitian plays an active role in the education of dietetic interns and Home Care staff. Responsibilities also include participation in provincial and local committees. Travel to clients’ homes within Calgary & Area is required.
Baccalaureate degree from an accredited university with a major in Foods and Nutrition. Completion of an accredited Dietetic Internship. Current registration with the College of Dietitians of Alberta. Minimum 4 years demonstrated ability and experience in clinical dietetics required. Experience working with seniors required. Recent home care experience required. Outpatient and/or community experience is desirable. Competency with computer applications and office programs required. Experience with multi-disciplinary teams, inter-sectoral systems, and/or community based services desirable. Experience providing consultative services to health care professionals preferred. Strong communication skills required. Must have valid AB driver’s license; vehicle required.
