Reporting to the Area Manager on the Seniors South Team within Integrated Home Care, the RN Interventionist/Case Manager (for acute/rehabilitation clients) works as a member of an inter-professional health care team.

Within the full scope of the professional practice, is responsible for assessing, planning, organizing, reviewing and evaluating the needs of Acute and Rehabilitation clients and their caregivers/families requiring community based services that fosters and promotes continuity of care and co-operative partnerships by liaising with primary care physicians, residential care facilities, acute care hospitals, and other programs/organizations involving provision of services and may work in multiple settings.

The RN Interventionist/Case Manager (for acute/rehabilitation client groups) is also responsible for providing direct nursing interventions to a select group of Long Term Supportive, Maintenance and End-of-life clients in a designated area on the Seniors South Team-Integrated Home Care Program.
  • Baccalaureate degree in nursing is preferred; however, skills and experience relevant to nursing practice in community sector will be considered.
  • Current registration with CARNA.
  • BCLS- HCP required.
  • Minimum two years of recent experience, previous community experience in geriatric an asset.
  • Gerontology certificate an asset.
  • Excellent interpersonal, communication and time management skills.
  • Proven critical thinking, decision-making and assessment skills.
  • Ability to monitor financial accountabilities.
  • Physically able to perform all assigned tasks.
  • Ability to perform all duties in accordance with related legislation.
  • Basic computer skills are an asset.

    Please note this position requires a vehicle for business use; possession of a valid driver’s license and access to a motor vehicle in good repair for work-related travel is required.
