Under the supervision of the Program Manager, Chronic Disease Management in Primary Care and Diverse Populations, this position is responsible for providing Chronic Disease Management with a focus on obesity care to the Aboriginal population in Calgary. The nurse provides coordination and integration of care, education and clinical management to Aboriginal population in a manner that is responsive to the cultural, social and financial circumstances of the target population. The nurse will utilize an organized approach to problem solving and is responsible to assess, consult and mobilize available interdisciplinary and community resources to facilitate Chronic Disease Management (CDM) care. Responsibilities include assessment, critical thinking, ongoing patient case management and, coordination of services for patients with chronic diseases. Additional responsibilities include facilitating communication between the project team and family physicians, community engagement and outreach, and data collection.
Key Responsibilities: The nurse will identify the real/potential risks at the bio-psycho-social-cultural-spiritual levels of persons at risk for or with a chronic condition. The individuals need for monitoring and surveillance, over what period of time, at what frequency and intensity and by whom to help people address life challenges in order to optimize health and wellbeing is established by the nurse. Based on an ethic of caring and the goals and circumstances of those receiving nursing services, registered nurses apply nursing knowledge and skill to: Assist individuals, families, groups and communities to achieve their optimal physical, emotional, mental and spiritual health and well-being. Assess, diagnose and provide treatment and intervention and make referrals to the most appropriate health care services, Teach, council and advocate to enhance health and well being, Co-ordinate, supervise, monitor and evaluate the provision of health services of Aboriginal patients with chronic conditions/obesity Coordinates the workflow and patient care in the clinical site In partnership with patients, develops the plan of care including consideration of cultural, social, financial and emotional risk factors, options, consequences of actions, health promotion, learning, comfort, safety, privacy, dignity and integrity. Reviews and revises care plans as necessary Collaborates with members of the health care team and individuals/communities to collect, validate and expand assessment data Communicates relevant patient information to appropriate physicians and caregivers while ensuring communication of confidential information only to authorized persons. Teaches Lifestyle and disease management classes to Aboriginal patients and high-risk individuals in group educational sessions in community-based sites. Collaborates with community organizations to explore innovative ways to promote healthy lifestyles considering the unique cultural, social and financial circumstances of the target population. Coordinates care/services with other team members and external agencies Acts as patient advocate to protect patient rights, beliefs, and values. Shifts are primarily days, Monday to Friday, occasional early evening, weekend shifts may be required and start times may vary.
Graduate from an approved Nursing program. Baccalaureate of Nursing required. Current successful completion of a BCLS program is required. Current registration with the Alberta Association of Registered Nurses of Alberta (CARNA) Additional courses or certifications specific to the practice environment and clinical population may be required. Previous chronic disease or community experience is an asset. Minimum of 2 years experience preferred. Current knowledge of community services and resources Knowledge of Chronic disease, obesity and Aboriginal culture is preferred Previous work experience with Aboriginal populations is required. Experience facilitating group and 1:1 patient education following adult learning principles preferred Demonstrates ability and current experience working as a member of a multidisciplinary team and to request and accept direction as required. Demonstrates strong assessment and critical thinking skills, including clinical judgment and problem solving. Demonstrated strong leadership, organizational and interpersonal skills Demonstrates initiative and ability to work independently in unique environments. Basic computer skills required
