Minimum: 34.31 per hour Maximum: 45.03 per hour
Supportive Living (SL) provides personal and professional care services for 3,000 residents in over 115 supportive living sites in the Edmonton Zone. SL is recruiting Case Managers to coordinate the placement, on-going assessment, care planning and delivery of professional and personal care services to our residents. Case Managers are also responsible for resident engagement, monitoring and evaluating care and outcomes, formal reassessment and disengagement. Case Managers facilitate the achievement of resident-based goals and applicable standards, and assists site-based nursing and support services teams through excellent communication, leadership, interpersonal and conflict resolution skills. Case Managers ensure collaboration within the site nursing team, the residents and their families. Case Managers function as leaders, advocates, educators, and stewards of resources and rely extensively on clinical information systems and computer technology to complete assigned tasks.
  • A Baccalaureate Degree in Nursing is required; equivalent experience may be considered.
  • Three years of recent nursing experience including geriatric experience (obtained within the last five years) or completion of a refresher program within the last two years may be considered. Previous community-related experience a definite asset.
  • Active registration with CARNA and Basic Life Support (BCLS-HCP) certification are required.
  • Possession of a valid driver’s license and a motor vehicle for work related travel.
  • Demonstrated ability to provide comprehensive assessments individuals within their physical and social environments.

    Must provide proof of active registration with relevant college and maintain registration throughout the duration of employment.

    • Vehicle Required - Required to provide a vehicle for business use; along with acceptable driver's abstract and proof of business insurance
