Minimum: 34.31 per hour Maximum: 45.03 per hour
If you are interested in playing a key role in linking the needs of Home Care clients and service delivery, this position will be of interest to you. Using your strong assessment and organizational skills, you will identify the needs of the client and determine the appropriate internal and exteranl resources. Through the development of the care plan, in collaboration with the client/family, you will ensure services are provided and modified on an ongoing basis, based on the client's response to the interventions.
Diploma in Nursing required. BScN preferred; however, skills and experience relevant to nursing practice in community sector will be considered. Recent care of community clients is preferred. Recent experience in the care of Acute Home Care clients is an asset. Current Acute Home Care skill certifications an asset. You must have current and clear registration with CARNA, BCLS-HCP certification, a vehicle, and a valid drivers license.

  • Vehicle Required - Required to provide a vehicle for business use; along with acceptable driver's abstract and proof of business insurance
