Children's Home Care is a multi-disciplinary team with RN, SW, RT, PT, OT, SLP and Rehab Assistants who collaborate and conduct home visits to deliver services to children from 0 - 18 years old with a wide variety of conditions, including children with complex health needs. As an RN for Children's Home Care, you will act both as a Case Manager as well as a direct care provider, utilizing your strong skills in family-centered practice, coaching, assessing, organizing and communication.

The Case Management duties include ongoing assessing and identifying the overall needs of the child and the child's family, in collaboration with the family and other partners and developing a care plan based on those needs. The care plan ensures appropriate services are provided, and the appropriate internal and external resources are accessed. Case Managers work within a Competency Framework that includes personal and professional accountability, systems thinking, creative thinking, communicating and building relationships.
A Baccalaureate degree in nursing (BScN) is preferred, however; skills and experience relevant to nursing practice in the community sector will be considered. Preferred: two years of current nursing practice in the care of Pediatric clients, computer knowledge/skills including Edmonton Zone version of Meditech and strong interpersonal skills. You must have current registration with CARNA, BCLS-HCP certification, a reliable vehicle and valid driver's license.

Responsible to provide proof of active registration with the College & Association of Registered Nurses of Alberta (CARNA) and to maintain registration throughout the duration of employment.
