*Remote Retention Allowance: In addition to a competitive rate of pay, AHS currently has a Remote Retention Allowance in place to an annual maximum of $3,000. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.

Reporting to the Area manager, Seniors' Health, the Home Care Case Manager provides restricted activities and care within the full scope of practice as defined by the College and Association of Registered Nurses of Alberta (CARNA).The Case Manager utilizes the nursing process, critical thinking and problem solving skills to provide care to individuals, families, groups and communities across the life span. The Home Care Case Manager ensures the development, implementation and ongoing administration of the client care plans. The Home Care Case Manager assesses, coordinates, supervises, administers and evaluates the provision of health services and healthcare resources. Through education and the interpretation of evidence-based healthcare practices and processes, the Home Care Case Manager plays a key role in support of an integrated and holistic approach to the client’s health care, health promotion and health maintenance requirements. Providing clinical leadership, with complex or unpredictable clients and client care settings, the Home Care Case Manager acts as the liaison between the inter-professional team and other providers of care to support the patient/client. The Home Care Case Manager provides professional nursing care using the nursing process, participates in the inter-professional client care process, and performs duties in accordance with the vision, goals, and standards of Alberta Health Services.

Current CARNA practice permit

Graduation from an accredited nursing program.

Baccalaureate degree in nursing preferred; however, skills and experience relevant to nursing practice in community sector will be considered;

If applicants with a baccalaureate degree in nursing are not suitable, applicants with a diploma in nursing and three years recent experience in home care or acute care may be considered.

BCLS required.

Valid drivers license and access to a reliable vehicle; Acceptable drivers abstract; Proof of business insurance;
Computer Proficiency;

Certification in gerontology, palliative care, wound care and/or foot care an asset.

Knowledge of nursing standards/protocols as required in area of specialty.

Knowledge of basic nursing care procedures required.

Knowledge of scope of practice as defined by CARNA.

Knowledge of the Canadian Nurses Association Code of Ethics.

A current and original Criminal Record Check is a requirement of all external applicants in order to be considered for a position.
