Minimum: 24.44 per hour Maximum: 32.00 per hour
Working in the (8 bed) Long Term Care wing of St Theresa General Hospital as a member of the health care team, provides direct nursing care as delegated by authorized personnel. The Licensed Practical Nurse provide direct nursing care as delegated and within the regulations and guidelines of the C.L.P.N.A; provides practical care including the promotion of wellness, prevention of illness, and to maintain and restore health and social well-being; is aware of and complies with the scope of practice, regulations and guidelines as set forth by their registering body; where acuity/complexity of patient/client requires, or as directed, confer with other professionals accountable for clinical leadership; participates in case conferences, nursing care and discharge planning.

An LPN is accountable for functions/processes of Single Entry Intake including providing information about continuing care services and initiating referrals; the LPN is accountable for the nursing care provided within the knowledge and skills outlines by legislation and the profession; uses clinical skills to assess the client’s health status to enable mutual planning, implementation and evaluation of appropriate intervention; manages all aspects of client intake process, including establishing and maintaining positive relationships with clients, families and referral sources; responds to customer requests and concerns; collaborates with interdisciplinary team members, educational resources and health care professionals/providers to achieve quality client outcomes; confer with other professionals accountable for clinical leadership where acuity/complexity of clients requires, or as directed; perform other duties as required.
  • Must be a graduate of a recognized L.P.N. Program
  • Current B.C.L.S. certification required
  • Active or eligible for registration with the C.L.P.N.A.
  • Must have 2 years experience with Continuing Care
  • Experience preferred in an acute care, home care, continuing care or preventative health setting, dependent on assignment
  • Proven written/oral communication skills
  • Demonstrated computer knowledge required, experience with MDS and RAI assessments an asset
  • Required to work both independently and as a team member in a multi-disciplinary environment
  • Knowledge of community resource an asset
  • Valid driver’s license and own vehicle as required

    • Vehicle Required - Required to provide a vehicle for business use; along with acceptable driver's abstract and proof of business insurance
