Working in Diabetes Specialty Care within Primary Care & Chronic Disease Management you will participate in the development, implementation, ongoing evaluation for quality Diabetes Specialty Care in accordance with AHS Chronic Disease Management initiatives.

The educator also supports the development, implementation, ongoing evaluation and quality improvement initiatives associated with diabetes care.

The educator works collaboratively within an inter-disciplinary team, with other health care professionals, primary care networks, community agencies and key stakeholders to further develop, explore and implement strategies to address diabetes care.

Self-motivated and results orientated, you possess expertise and competence in clinical practice, leadership, counseling, teaching and presentation skills for chronic disease management - in particular diabetes care management. Excellent diabetes management skills related to pregnancy and diabetes, the pediatric client and intensive diabetes management skills including the initiation and management of insulin pump therapy is an asset. You possess excellent communication skills; can work independently and collaboratively on a team.
Current registration with CARNA, a baccalaureate degree in nursing is preferred along with 3 years clinical experience. Non-degreed graduates may be considered on equivalent basis with 3-5 years of recent experience in the area of chronic disease management.
  • Certified Diabetes Educator through Canadian Diabetes Educator Certification Board as asset
  • Extensive knowledge of chronic disease care in particular diabetes care, modifiable risk factors and lifestyle management
  • Demonstrates problem solving, motivational, goal setting and group facilitation skills
  • Strong networking and communication skills both verbal and written
  • Excellent organizational, time management, initiative, accountability and critical thinking skills
  • Ability to work independently and collaboratively as part of team
  • Current Basic Life Support BCLS-HCP
  • Motivational Interviewing Skill (e.g. Choices and Changes or Health Coaching)
  • Proficiency with advanced computer operations is essential including Meditech, the Internet, Microsoft office programs and audiovisual equipment is essential
  • Valid Driver's License required
